Responsibilities
Conduct comprehensive assessments and ongoing reassessments of clients, including psychosocial, physical health, mental health, environmental, functional, and spiritual needs.
Write comprehensive assessments and, based on assessment information, collaborate with the client to develop and initiate a Community Living Plan that is client-centered, comprehensive, and consistent with program guidelines and policies and procedures.
Conduct home visits, community-based visits, acute hospital visits, and skilled nursing facility visits, prison visits, jail visited, and other relevant locations to justice-involved individuals, and escort clients to medical and other appointments as clinically indicated.
Identify, arrange, and monitor appropriate community services based on strong working knowledge of Medi-Cal, Medicare, and other entitlement programs, with particular attention to services supporting reentry from incarceration.
Establish and maintain care management relationships with clients and, as appropriate, their informal support networks, offering respect, dignity, and support, and providing crisis intervention, advocacy, problem-solving, and therapeutic interventions.
Meet with clients at least monthly, and more often as clinically indicated, reviewing and modifying the Community Living Plan on an ongoing basis.
Document all case management activity through progress notes within 2448 hours, adding new problems and interventions to the Community Living Plan as needed.
Maintain required paperwork and a clear, concise, and consistent system of charting to support continuity of care and regulatory compliance.
Conduct ongoing evaluation of Purchase of Service needs and follow up to confirm services are provided in a timely manner.
Educate clients and informal support networks about available resources, benefits, and community-based supports, including reentry-specific services.
Establish and maintain open and effective communication with community providers, including correctional health partners, physicians, behavioral health providers, housing providers, and social service agencies, while maintaining confidentiality.
Monitor the quantity and quality of services provided by involved providers.
In collaboration with the interdisciplinary team, continuously evaluate clients ability to remain safely housed in the community and coordinate placement when appropriate in accordance with program guidelines.
In collaboration with clients, caregivers, and involved services, appropriately terminate services when indicated and document the process as required.
Participate in research studies, data collection, and Continuous Quality Improvement activities as required.
Attend and actively participate in team and program meetings, activities, supervision, and problem-solving efforts, contributing to open communication within the team.
Utilize supervision appropriately, maintaining open communication regarding caseload activity and client needs.
Actively incorporate ethical and legal standards of the National Association of Social Workers into all aspects of work.
Understand and apply Institute on Aging regulatory and procedural requirements.
Attend continuing education and in-service training to enhance knowledge and skills related to case management, reentry services, gerontology, behavioral health, family systems, and community resources.
Perform all other reasonably related duties as assigned.
Education
M.S.W. (Masters in Social Work) or another appropriate Masters level degree such as an MPH, MFT with additional or specialized work experience such as psychology, counseling, or geriatrics.
Alternatively, in lieu of a Masters degree, an employee may qualify for a Care Manager II position with a BA or BS in Social Work or another appropriate major and a minimum of two (2) years of relevant social work experience and the ability to demonstrate autonomous work in conceptualizing and formulating biopsychosocial assessments, identifying care needs and necessary interventions, and then executing effective care interventions.
Background and Experience
One year of experience working with disabled adults and/or older adults required.
Experience working with justice-involved individuals, individuals with mental and/or behavioral health diagnoses, and/or substance use disorders strongly preferred.
Experience with and understanding of the medical and psychosocial needs of functionally impaired adults and older adults.
Demonstrated case management experience within community-based or healthcare delivery systems.
COMPENSATION
Range: $84,000- $92,000/Annual
