POSITION TITLE:Retention in Care Manager

CLASSIFICATION:Exempt

POSITION DESCRIPTION:The Retention and Care Case Manager (RCCM) resides within the Housing & Health Navigation department and is a part of the Housing & Health team. They will employ a health-system based and integrated health and social care service delivery model to improve overall medical, behavioral health, and social care outcomes for unstably housed people living with HIV and those vulnerable to HIV acquisition. The RCCM is part of a team that consists of medical and non-medical care staff. They may be required to work non-traditional business hours to meet the demands of the program. The RCCMs primary role is to provide case management services focused on social care to support the rapid linkage and retention in integrated health and social care for unstably housed people who are newly diagnosed, out of care, at-risk of HIV, and those who are chronically disengaged from medical care and housing supports. Comprehensively, they will be addressing the multiple barriers to accessing and staying retained in integrated health and social care.
