ABOUT US:Legacy LA is a community based non-profit organization focused on youth development. Founded in Ramona Gardens, Legacy LAempowers youth and families in Boyle Heights and surrounding neighborhoods to break systemic cycles of multigenerational poverty and trauma through leadership, healing, and community-driven change to build resilient and equitable neighborhoods.

SUMMARY:The case manager for the GRYD Program will provide academic and behavior intervention services to high-risk youth who reside in Hollenback 2 GRYD Zone, which includes Ramona Gardens Housing Development and the Boyle Heights area. Utilizing a holistic approach, GRYD case managers address complex personal and academic challenges faced by youth; and support them in overcoming these obstacles through intensive mentoring, goal setting, tailored intervention methods, parent/guardian meetings, referrals to necessary resources and comprehensive assessments. GRYD Case Managers work closely with the Middle School Program team including Program Coordinator, tutors and program staff to aid in program development and implementation.

DUTIES AND RESPONSIBILITIES:

  • Case Manage 25-30 youth using GRYD model and framework, 20 Model/10 Primary
  • Stay informed of and adhere to GRYD framework and expectations, including but not limited to:
    • Administer assessment (YSET) to determine program eligibility
    • Facilitate two, one-hour face-to-face family meetings, for 20 model prevention clients
    • Conduct one individual, 1 hour, meeting for 20 model prevention clients per month
    • Conduct one strategy session, of 30 min, per month for each model prevention client
    • Face-to-face meeting with 10 primary prevention client, 1 hour per month
    • Contact parents of primary prevention client, once per month for 1 hour
  • Support with the creation and implementation of programs in alignment with GRYD-AIYDA framework
  • Participate in GRYD & Legacy LA meetings, trainings and events as needed (weekly/monthly/quarterly)
  • Connect with school personnel and community partners to support youth and families, provide school site visits, and foster relationships with potential youth
  • Identify and document individual, family and group activities; keeping track of youth and family strengths and utilizing them to reduce risk factors and challenge-behaviors
  • Maintain physical case files and electronic records using ETO database to track client progress.
  • Support organizational outreach, fundraising and special event efforts, including but not limited to: overnight retreats and trips, community events as needed.
  • Build strong relationship and communication with program manager, case managers, tutor coordinator, to ensure quality service delivery
  • Support programmatic efforts agency-wide as needed.

PREFERRED SKILLS AND ABILITIES/EXPERIENCE AND EDUCATION

  • A Bachelors Degree in social work or related field from an accredited college/university is preferred
  • 2 years of experience in youth development. Previous GRYD experience preferred
  • Excellent interpersonal skills including oral and written communication skills
  • Ability to work effectively with diverse clients with a broad spectrum of assets and needs
  • Strong computer skills, Microsoft Office Suite; ability to create and maintain the resident database and to find resources on-line
  • Basic Social Justice Framework understanding
  • Willingness to support in transporting youth in van as needed
