The Greater Community AIDS Project (GCAP) is seeking a compassionate, motivated Case Manager to support clients transitioning into permanent supportive housing while connecting m them to essential healthcare, employment, and community resources.

What Youll Do:
- Client Engagement & Goal Planning
- Case Management & Advocacy
- Documentation & Compliance
- Outreach & Support
What You Have
- Motivational interviewing and strengths-based case management experience
- Trauma-informed care approach
- Strong communication, organization, and documentation skills
- Crisis de-escalation ability
- Valid drivers license, reliable transportation, clean driving record, and willingness to transport clients
- Bachelors degree in social work, Human Services, Psychology, Sociology, or related field preferred OR Bachelors degree in another field with at least 20 semester hours in human services/social services
