Your Opportunity Mission Australia is seeking a skilled and passionate Case Manager to join our team, based in Tweed Heads or Lismore and supporting clients in Tweed and Byron Bay. This is a meaningful opportunity for professionals experienced in working with vulnerable children, young people and their families. In this role, you will provide trauma-informed, early intervention case work. You will deliver a mix of long-term and short-term support services, including school and/or home-based case management, needs assessments, practical assistance, referrals, and group-based interventions. You will also participate in community outreach supporting broader awareness and access to services. Were looking for people with a strong experience working with in child and adolescent mental health, trauma-informed practice, and family-focused care, who are confident engaging with diverse communities, including Aboriginal and Torres Strait Islander and CALD families. Your key responsibilities will be to: Deliver trauma-informed, client-centred case management to children, young people (018), and families experiencing or at risk of mental health issues. Respond to referrals, conduct initial needs assessments, and provide ongoing support through home visits and outreach services. Develop and implement individualized case plans, provide practical support, and facilitate referrals to internal and external services. Coordinate and deliver therapeutic and psychoeducational programs, including Play Therapy Groups and mental health education. Maintain accurate and timely case documentation, including risk assessments, progress reports, and data entry. Work collaboratively with internal teams and external agencies to advocate for clients, participate in case reviews, and support community development. Requirements for Success: Tertiary qualifications and/or relevant experience in the human services industry, with an understanding of mental health. Provide tailored case management through comprehensive assessments, developing individualized case plans, and facilitating ongoing support for clients. Respond to client referrals promptly, ensuring timely contact, support, and effective service delivery to children, young people, and families. Coordinate and deliver individual and group support programs, such as Play Therapy Groups, mental health education, and community outreach. Excellent oral and written communication skills, with the ability to maintain accurate case documentation and engage effectively with clients and external agencies. Current NSW drivers license Current First Aid and CPR Certificate Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
