Find out more about us and our purpose. Your Opportunity Looking to make a real impact with young people? Join us as a Case Manager and help high school students re-engage with learning and life! Youll work one-on-one with youth aged 1521 (or 25 for young parents), creating personalised learning plans and providing the support they need to thrive. Your key responsibilities will be to: Respond to referrals of clients to the service from internal and external support services and conduct over-the-phone and formal face-to-face assessments of suitability for support Register and induct clients into the service including the property, facilities, financials and regulations Provide ongoing case management sessions (formal and informal) with clients and review progression against case plans and provide informal counselling as required Assist clients in the process of transition out of the service into independence or other services Development of an agreed individual learning plan (ILP) in conjunction with (if needed) an agreed case plan including goals and outcomes Develop strong internal and external relationships with clients, staff, and key stakeholders to contribute to the effective functioning of the service and improved outcomes Requirements for Success Certificate IV or Diploma in Youth Work or Youth Justice, Bachelor of Psychology or Bachelor of Social Work Previous experience working in a case management approach within a education or employment focused environment Previous training in providing a Child Safe Environment, Reporting Abuse & Neglect and Education in Care RRNH-EC certificate (4 hour master class session) Current Working with Children Check Current Drivers Licence First Aid Certificate RRNH-EC certificate (4 hour master class session) Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
