Position Overview
TheCase Manager Counselor Reentry Servicesplays a critical role in supporting individuals returning home from incarceration. This position providesholistic case management, career counseling, and reentry planningfor participants preparing for release from correctional institutions and continuing support after release.
The Case Manager will work directly with incarcerated individuals instate correctional institutions and county facilities, helping them develop plans for employment, housing, education, and community stability.
Key Responsibilities
Pre-Release Case Management
- Conduct assessments with incarcerated individuals preparing for release
- Develop individualized reentry and career plans
- Provide program orientation and enrollment support
- Coordinate with correctional staff and reentry partners
Post-Release Support
- Provide continued case management after release
- Assist participants with employment readiness and job placement
- Connect participants to housing, healthcare, and community resources
- Support participants in meeting probation or parole requirements
Community Collaboration
- Work with workforce development partners, employers, and service providers
- Facilitate referrals to supportive services
- Participate in team meetings and program reporting
Qualifications
Education
Bachelors Degree in Social Work, Psychology, Counseling, Criminal Justice, or a related field.
Experience
- Experience in case management, social services, or workforce development
- Experience working with justice-involved populations strongly preferred
Additional Requirements
- Valid drivers license and reliable transportation
- Ability to travel to correctional facilities across Pennsylvania
- Ability to pass required correctional facility clearances
