Position: Outreach Worker

Location: Portland House, 243 Shalesmoor, Moorfields, Sheffield S3 8UG Working Hours: 37 hours per week Monday to Friday Contract Type: Permanent Salary: 25,867 – 32,090

About the role:

We have an exciting opportunity to join our HALT Team (Home at Last) , supporting adults who are rough sleeping to access treatment and accommodation across the city.

This role is part of a multi-agency approach, working closely with housing providers, homelessness services, and other professionals to deliver coordinated support.

You will be engaging with individuals in the city centre and across wider areas, offering outreach-based interventions to support people experiencing multiple disadvantage.

The position involves proactive engagement with individuals who may be facing complex challenges, including homelessness, substance use, and other vulnerabilities.

You will work to build trust, provide practical support, and help individuals move towards stable accommodation and access to appropriate treatment services.

As directed by the Project Manager and Lead Practitioner, you will:

Deliver outreach interventions to individuals rough sleeping or at risk of homelessness

Support individuals to access accommodation, treatment, and wider support services

Work collaboratively with housing providers, homelessness teams, and partner agencies

Promote harm reduction and public health approaches where appropriate

Contribute to reducing the impact of multiple disadvantage and improving outcomes for service users

Please note, this role includes some weekend work and occasional evening shifts.

About you:

Proven experience of engaging effectively with people with complex needs

An understanding of the challenges faced by individuals experiencing homelessness and/or substance use

Experience completing service user assessments, including risk assessments

Ability to work collaboratively within a multi-agency environment

Good IT skills

A full driving licence and access to a vehicle
