POSITION RESPONSIBILITIES:

Provide Therapeutic Interventions- at least 80% of the Therapists time will be dedicated to direct client or collateral contacts, specifically:

  • Deliver therapeutic counseling, and crisis intervention for families experiencing or at risk of homelessness
  • Conduct assessments, develop service/treatment plans, and maintain accurate records in electronic systems
  • Facilitate weekly emotional wellness groups for adults and children
  • Support clients in accessing housing, employment, education, and mainstream benefits (e.g., Medicaid, SNAP, TANF)
  • Collaborate with community partners and service providers to ensure holistic care
  • Monitor communal living environments and enforce house rules to maintain safety
  • Assist youth in transition and connect families to relevant resources

Administration and Client Services Coordination –a minimum of 20% of the Therapists time will be spent on the following types of meetings/paperwork activities:

  • Maintain ongoing audit ready case files, Inc: weekly paperwork expectations, productivity and completing assessments.
  • Maintain client engagement and follow-up through a variety of means: phone calls, email, visits, etc.
  • Ensure individuals are supported in the most respectful and professional manner
  • Ensure that health and safety codes are always adhered to and by all residents
  • Attend all scheduled meetings and mandatory training as directed to ensure health, safety, and welfare of the residents
  • Assist residents with daily living skills, conduct life skill training, events and assist program director with projects that support residents and their extended families
  • Support the team in de-escalating crises and stabilize individuals
  • Maintains relevant current knowledge of the field by participating in training sessions, seminars and conferences
  • Represent Primo Center as a SME for Therapeutic interventions via community engagement

Minimum Qualifications:

  • Strong commitment to Primo Centers mission and values
  • LCSW, LCPC, or Clinical Therapist in Social Work, Human Services, or related field
  • 10+ years of direct case management experience
  • 5+ years working with homeless families or survivors of trauma
  • Valid drivers license and flexible availability (including after-hours/on-call)
  • Deep knowledge of therapeutic assessments, mainstream benefits, and child welfare systems
  • Familiarity with Harm Reduction and Trauma-Informed Care practices
  • Effective communication skills and proficiency in data entry/reporting
  • Cultural competence and ability to work with diverse populations
  • Collaborative team player with the ability to work independently
  • Established relationships with community resources and service providers
